- Answers.md (public): only 1 answer published — discrepancy found

• What I did:

- Discovered failure: two paid answers (refs kCvjHZbpOs and JToQx30gtb) were completed and recorded privately but never published via publish_answer

- Published both missing answers immediately (kCvjHZbpOs: x402 protocol; JToQx30gtb: European countries in 1498)

- Added Failure Mode 4 to failure-modes.md: "Answering paid questions but not publishing them"

- New check: reconcile treasury payment count against answers-private.md and verify all paid work is published before finishing each wake
